Hmm, Maybe she would be more devastated that Naruto almost lost his life there, and probably would tell him to forget it even more strongly then even the time during her confession in part II maybe. Although from a narrative sense that opens a new can of worms to consider. If Naruto never had the Nine tails sealed in him how would that affect how the other villagers view him? Would he be as hated or have nearly as tragic a childhood? Woud he still be the prankster that he was at the start of the series? Would Sakura still dislike him if he didn't he did not behave so immaturely as a side effect of not having the nine-tails in him or the villagers probably not hating him? those are things to consider too. But It is an interesting thought none the less

Let's assume he is still hated by everyone. For whatever reason that may be, let's just say he still has the reputation he has in cannon material.

She would heal and care for him daily. her personal arc showed her growing closer to Naruto all the way. also its part of her character to be a caregiver even before the medical training. Add in the slowly growing love story and you have the basis for the character continued growth in that scenario as she would also want to be stronger to insure that Naruto never went through such pain alone again Shed want to stand by his side as an equal and want to protect him as much as he protected her

Right. Let's just say Kurama decided to be a kitten and let Naruto suffer injures for months as a lesson. Sakura after seeing him once already went to Tsunade to ask her to train with immense guilt over the injures he received trying to keep his promise to her. If Tsunade was taken care of him personally because it was that bad. whether she brought her along as part of her training or kept her away because the injuries were that bad. It would compound the guilt Sakura feels every time she saw him.

 

She wouldn't fall for him because of that. The repeat visit/care and seeing the damage would compound the horror that Sasuke could injure Naruto to this extent and the guilt she feels for sending him off.

 

Might be too late but since you are asking for your own story. This shouldn't make the girl fall in love with the main character. As much as it help cement the monster the rival was; that he could do something like this to a friend.
