Most I've seen are either after the fact (Sakura doesn't love Sasuke anymore) or an AU where she never really loved Sasuke and only Naruto. Are there any good ones where it deals with Sakura's conflicting feelings properly and she chooses Naruto in the end?
Any good fanfics that's SS then NS?

There actually are several, but it's usually some kind of ridiculously depressing story where Sakura chooses Sasuke because Naruto never told her about the bench scene and Naruto becomes a miserable sack of bitterness instead of talking to Sakura. Or her feelings are handwaved like you said.
Basically none of them were done in the way I would have wanted, so I ended up writing one a while ago though I dunno if it's exactly what you are looking for
