Well if you read what my friend told me, then she told me 'otto' is a way to mark a distance with your husband.
As far as I know otto means simply husband
but since sasuke was out of the village for some years this makes a bit of sense
(i think the "distance" you're saying is the fact they don't saw each other for some years)
