Genjutsu or not they're still real. She gets thrown about, cut up and even orochimaru stabbed her in one. Thinking about them when she reverts to original form....they shrivel up.

To make her loom young yes but she does have those actual size breasts.

I thought she had on a chunin vest on when Dan died, oh well I could be wrong ^^; But aside from that her breast size when she was younger were still pretty big. She may have a gen-jutsu to make her look youthful which would make them more perky but their size would be the same without the jutsu, just saggy. Now here's my take on how she does everything with her big tatas. For starters she's always had them for a great deal of her life so their size shouldn't be an inconvenience. Shes had them for a good amount of time so she's use to fighting with them. Now if you were to give Sakura that size of breasts right off the bat there would be problems. One thing I do think Tsunade must have is some back aches, but when in battle for your life something insignificant like that isn't important.

It's not so much an issue of weight, but more like.. they get in the way XD
I've got a pretty big bust, and i've been doing martial arts for 10 years. After I hit 16 I remember having to wear 2 sports bras to keep these buggers down.

They really do get in the way, and it's distracting.. for both parties XD and it hurts!
Freaking boobs flyin; all over the place. it actually hurts lol.

This conversation reminds me of how Jiraiya said to Tsunade, during their last outing before he died, that she keeps all of her sadness within her breasts and gave it as a reason as to why she went from flat chested to endowed. He even said that from then on they might get even bigger, as if he was already predicting his own possible demise in that mission. The whole breast comment is funny and sad at the same time.

Maybe it's just me but I think Tsunade's breasts got a little larger after Jiraiya died. Goes to show how much she actually cared for him.

As for how her breast remain in place, I don't really give it much thought and pass it off as manga logic.

Hahaha well I can imagine how big breast can get in the way even though I've never had the same problems with mine. As for Tsunade... maybe she uses them as a weapon? They could be for distracting opponents or maybe she can also hit with them?

I think it's more like a permanent transformation than a genjutsu.
The transformation fades when she's empty of chakra, and heck, she looked more than 54 years old in coma. XD (do not tell her )
