Only games I can think of that would count are perma death games like FTL, Binding of Isaac, and Dungeons of Dredmore. Of course with all 3 of those I had to die and restart from the beginning before winning. Generally any game that can be beaten first try without any sort of failure state is too easy in my opinion. It's why I generally start at the highest difficulty in games and work my way down if things become unfair.
One game in particular I feel like mentioning is Xcom Enemy Unknown. I played through that game on the 2nd hardest difficulty on Iron man. It had to be one of the most stressful and rewarding experiences I've ever had in a game. Technically I lost a few soldiers but I beat the campaign without failing a single mission so I count that as "without dying".

Once I beated God of War 2 on titan without dying a single time. By the end of the day, I was both feeling very acomplished and reduced to a complete nerve wreck. Good times...
And Dragon age Inquisition. Dem knight enchanters, feels like god mode.

Freedom of speech is no laughing matter, since it does take one person to stand up for the right too speak out.
