One way that I can see things still going in the direction of the OVA is that when Sakura arrives and starts healing her boys, she spends most of her time by Naruto’s side. While she checks that Sasuke’s recovery is going well, she mostly sits by Naruto, holds his hand, and even is on the verge of tears as she sees him in pain at times. Naruto can squeeze her hand back and give her reassuring smiles that he’ll be okay. It might even open the door to him being able to confess, especially if Sasuke, who is smart and observant, catches on to these little things and even gives a little bit of a push.

Kakashi will remove his mask and use it as a tourniquet on Sasuke's arm so he won't bleed out while Sakura takes care of Naruto.

 

Naruto, in a woozy daze, either confesses to Sakura or says he wants to kiss her forehead -- which will trigger her memory of #3.
