How did it happen? Did he mishandle his own rifle or fire-arm, or was this a 'movement-under-fire' course, where he had to be able to work under live-rounds?

it was a react to an ambush training platoon level. so you had around 30-40 guys maneuvering with live rounds. what happened was that as his squad was the assaulting element, they were using smoke signals for the sign to lift and shift fire. the smoke deployed late and as they came up on this hill the suppressing squad wasn't able to lift and shift fire in the appropriate time and the kid was the first one that came up over the hill. as soon as he was hit they called for a cease fire and canceled the rest of the training.

I'm reading Winter Melody (A D.Gray-man Doujinshi), watching The Walking Dead, reading up on historical countries for a game I play, and considering doing actual work on my fan-fiction or abridged series. Key-word considering.

Heh, thanks. My bat is a Louisville Slugger, 125K. Solid wood. I've got an axe in the garage, a BB Gun under my bed, and a set of long kitchen-knives in the kitchen. I'm fully prepared to mess up any fool/zombie who thinks they want a piece of me. :|
