Sasuke's forehead poke essentially means he is offering his arm to Sakura....to sew on naruto's stump. Naruto will have Sasuke's arm.

The thing is, Sasuke quoting his brother and doing the same thing with Sakura makes little sense. Itachi's gesture was an act of brotherly love. Sasuke would never initiate such contact with Sakura and I doubt she would be happy to get it after she was " stabbed through the heart" by him.( not to mention, he's really weak right now,)
I think the Itachi part of the pic is just to throw us off, since posting an NS one, would be too obvious.
