Well, the mask or image of the mask (not sure exactly what it is) is from HySe, so Uta's mask studio.

The book... well, either it's Hide, or someone else (I hope it's Hide cause not too many know about Sasaki's real birthday).

It could also be from Hinami/Touka. I'd honestly beg on Touka, Hinami might have left the book in her possession. Plus that would help fortify Touka's quote in this chapter, and possibly mean that she's indeed looking on him from afar.
