Well, talking business wise, most of the profits are coming from Japan, and if there's one thing we know it's that Japan likes NS. With that said, Kishi shouldn't have any trouble writing the story the way he wants to, regardless of what foreign anti-Sakura countries think. And yes that means all of us, since from what I've heard pretty much the entire world (except Japan) hates Sakura. Worst case scenario : Online complaints that only serve give free publicity.
And anyways, given that Kishi is also the mangaka of one of the big 3, I don't think he has to worry about money for a long long time.
The problem with the "popularity" argument is that the yaoi pairings are actually more popular in Japan, and Sakura and Hinata have always ranked similarly in the polls (and self-reported polls coming from only people who voted aren't very accurate in the first place). In other words, it is actually very difficult to discern to true popularity of a character or pairing, so it's best to leave them out of pairing debates altogether.
I think if kishi makes NS canon now he wont lose a major amount of his fanbase, remember, the japanese fanbase is his main fanbase, and narusaku (and not to mention sakuras fanbase) is popular in japan ans it looks like its gonna beat sasusaku judging by the events. Naruhina doesnt even exist (same goes to hinata), dont believe me? Go to japanese forums or whatever and look for a naruhina fanbase, or even on twitter for thre matter, goodluck with that mates 
No, before the narusaku event was announced, there had already been several sasusaku events. These events are purely fan-organized, anybody with enough money to rent a floor at a showroom can have one, and shouldn't be used to judge a pairing's popularity.
For me at least, I'm just content knowing that Naruto and Sakura have such a beautiful relationship that we don't need polls to back up our pairing.
I do think he does but I think Kishi doesn't make it relevant to plot or even to Naruto. I think it was made to be a bonus for fans as well as probably make a use for Sakura to feel "uneasy" with her feelings for Naruto. It should have been a big deal, but really, it's very minor.
I've seen several people point out that it seemed out of character for Naruto to just ignore a friend like that. Sakura's uneasiness towards Hinata's confession could be another barrier to her accepting Naruto's feelings, it would be interesting if Kishimoto did bring it up.
