It was a childhood crush but what about when that ninja asked her who she loves and she was thinking about Sasuke
Going by the MP translation "If its somebody you love then they're bound to be a great person" she thinks of Sasuke as a counter-example. We don't know who she was referring to originally it was left purposefully debatable and ambigious (otherwise the person she was thinking of whether it be Sasuke, Naruto or Lee) would have appeared in the background like she thought of in chapter 540.
I'm not sure what context the fans in Japan took it as since they're opinion would be more valid then mine via actually being able to read and understand the raw. But the context I got from it was that she was in love with someone (it doesn't show who) then fodder nin says that whoever she loves must be great person and she thinks of Sasuke as a counter-example as if not everyone she loves or loved is a good person. If we go by the context of Peirrot TV Sasuke probably wasn't the person she was referring to since a couple episodes afterwards she referred to him as "someone she used to love". On top of that you have the 09 interview with Kishi where he flat out says that contrary to what alot of people think, Sakura was honest with her confession.
Then we have Kakashi in 675 saying that Sakura feels its her duty to save Sasuke from darkness because he's someone she used to like (which i believe is in direct connection to 540) as if Kishi was trying to tell us Sakura feels sad in 540 because Sasuke (the person she used to like) is in darkness and she wants to save him from that much like Naruto does.
Everything that happened in the LOI arc was basically Sakura letting go of Sasuke romantically and almost putting him down like a rabid dog until Naruto filled her back up with hope that team 7 can be together again. She was getting over Sasuke in a romantic way but still wants to save him from darkness the whole thing was her acknowledging Naruto was the better choice.
