Doctor Kenzo Tenma is a young genius neurosurgeon in Germany in 1986. An accomplished brain surgeon, he seems to have everything, the favor of the hospital's director, Udo Heinemann, and Heinemann's daughter as his fiancee. However Tenma is dissatisfied in the political bias in treating patients in the hospital and is set to change things when he decides to operate Johan Liebert instead of the mayor of the Dusseldorf, who arrived later. Johan is saved however Mayor Roedecker dies. Tenma loses his standing and his fiancee as a result, but comforts himself by believing that he did the right thing and despairs on ever achieving his earlier high goals. However, Doctor Heinemann and the other doctors were found murdered.

Several years after, Tenma is Chief of Surgery at Eisler Memorial. A patient of his, being questioned in connection with the murder of middle-aged couples across Germany, is murdered in front of his eyes, and the killer is revealed to be Johan Liebert, the child who disappeared. Distraught on how the right thing became his worst nightmare, he decided to hunt down Johan, wanting to take the Monster down as an atonement of his mistakes.
